What is home health care?

                Home health care are services that are provided to the elderly or disabled. These services are usually provided through an agency or an individual, in a client’s place of residence. The purpose of such services is to help a person with an illness or injury. It enables the client to receive help during recovery, and help regain independence to become as self-sufficient as possible.

 

How often are services available?

                Our care services are available 24/7. A personal care plan will help determine the frequency and type of services that will be provided to you. Care plans are reviewed and updated annually to ensure the best care for a client’s needs. After reviewing and updating the care plan, type and frequency of services may increases or decrease.

Who is eligible for home health services?

                Most clients who receive services are elderly or disabled. A doctor will have to determine if you are eligible for services. Once determined we can begin the process to initiate services. If for any reason if we cannot provide services to you, we will help you find someone who can through our referral program.

What kind of services do you provide?

                We break up the services into two categories: general service and complex care. General service provides everyday care tasks. These tasks may include but are not limited to dressing, eating, mobility, housework, meal preparation and shopping. Complex care service provides medical based tasks. These tasks require training. Some of the tasks associated with complex care include but are not limited to bowel program, range of motion, skin care and wound care. This is not a complete list of all provided tasks. What is the process to have services provided to me?

                To help us spend more time on care we make the process for receiving services as simple as possible. The first thing that is done is initial contact. We become aware of your need for services usually through a referral made by a doctor or case manager. Sometimes clients contact us directly for services. In that first contact we gather basic information and set a day and time to do the initial meeting. We come to your home to meet with you and any necessary individuals to discuss your care requirements. During this time, we begin creating a care plan for you. This plan can be modified at any time. It allows us to know the exact services along with the days and times of services needed. Once you approve the care plan, we begin to find the best caregiver to match your needs. Once we match you with a caregiver services begin.

How soon can I expect to receive services?

                One of the things that are asked during an initial meeting, is how soon you would like to start services. Our most common reply is as soon as possible. We make this happen by following the process and making sure we didn’t miss anything. We have a comprehensive list of caregivers along with their specifications. This allows us to find the best match with your preferences. Services are initiated after a caregiver has been matched and introduced.

What happens in an emergency?

                Emergencies can happen anytime anywhere. We want to be prepared for that situation. Our staff and caregivers are trained to help you in an emergency. We will contact the appropriate individuals so you can get the help you need. We will stay with you and give you comfort as needed.

How much does home health care cost?

 

                More often than not home health care is covered through insurance. Depending on the insurance coverage you have, some or all of your care will be covered. If you don’t have insurance or the insurance you have does not cover care, don’t worry. Our administrator will give you options the best fit your budget. Contact our office for more information.
